A refusal is:

• A failure to appear for any test, other than a pre-employment test.

• For pre-employment, if an employee leaves once the testing process has begun, but before it is over.

• Failure to cooperate with any part of testing.

Always write remarks on the CCF documenting the refusal, and call the DER right away to notify them of what occurred.

If it is a refusal you do NOT send anything to the lab, but always send copy 2 to the MRO.
